Compensation (Pay): $19.11 to $27.71/hourly, based on experience.

Summary:  The Lead Referral and Resource Coordinator’s primary role is to support the Central Referral Practice Manager in a variety of projects and operations to include training support/ workflow troubleshooting with health center referral and resource navigators, assisting with the development of training plans and materials, centralized referral loop closure, tracking of staff performance, and assist in the development of efficient workflows in order to optimize the organization’s standardized referral process. Serves as a centralized liaison/ conduit with the Health Center Care Team, patients and referring service entities within the PVCHC service area. Enhances the effectiveness; efficiency and productivity of the department by contributing to, and participating in, departmental performance improvement goals and communicating feedback from clinical staff regarding referral workflows and associated processes and policies.

Essential Duties and Responsibilities include the following. 

  • Works in collaboration with Clinical Leadership Team and Referral Practice Manager to assist with the orientation, onboarding, training and familiarization of referral management workflows.
  • Provides Centralized or Organizational referral navigation, processing, and timely follow-up to include clinical, social, and other referred services.
  • Supports completion and/or closure of the referral and navigation process for all Peak Vista health centers in order to maintain continuity of care and compliance with all applicable regulatory requirements and internal policies and procedures.
  • Collaborates with clinical staff, patients, support teams, and community providers to ensure smooth transition to the most suitable level of care.
  • Knowledge of and ability to utilize all referral processes applicable to the payor source, including insurance pre-certifications and prior authorizations
  • Develops and maintains good working relationships with all departments, the care team, medical staff, and leadership team.
  • Develops and maintains positive community partnerships with health care referral sources and support agencies.

  • Travels to various health center locations to provide on-site training and support as needed.

Education:
High school diploma or equivalent education.

Work Experience:

Minimum 2-year experience in a healthcare or social service setting is required.

Computer Skills: Basic – ability to access the intra/internet to manage timecard, review policies and procedures, and read company communications; use e-mail to communicate with co-workers, leadership, and other departments; enter and correct data, modify a workbook, format a worksheet, and use printing functions; create a simple presentation in PowerPoint, run it, and print it.

Certificates and Licenses: CPR Certification to be obtained within the first 90 days of employment or when established as a requirement, if applicable to patient-interaction and collaboration with health center operations.
