Compensation (Pay): $55.69 to $80.75/hourly, based on experience. There will be a $4.00 per hour lift added to this position.

Job Summary: Licensed pharmacist with specialized advanced education and training who possesses the clinical competencies necessary to practice in a direct patient care environment. Provides comprehensive medication management and related care for patients. Assigned to work in one or more pharmacist-operated ambulatory care centers. Ensures that drug therapy needs of the patients are safely and effectively met. Supports clinic operations. May also work in prescription dispensing in the Peak Vista Community Health Centers Pharmacies.
Essential Duties and Responsibilities
- Performs all essential duties and responsibilities of a Pharmacist and Clinical Pharmacist
- Processes or delegates new patient referrals and dis-enrollments for ambulatory care centers in which they work. Ensures communication of patient status to the patient’s primary care team
- Oversees good faith estimate procedures for all pharmacy-operated ambulatory care centers
- Maintains competence in clinical problem-solving, judgment, and decision-making; communication and education; medical information evaluation and management; management of patient populations; and a broad range of therapeutic knowledge domains
- Practices in one or more pharmacy drug therapy management centers
- Develops meeting summaries as delegated for distribution to relevant staff following clinic meetings
- Educates colleagues on new treatments and evidence-based guidelines relevant to the specified clinic through clinical pearls, summary documents, or presentations
- Serves as a consultant to Peak Vista medical providers through direct communication or via educational programs with distinct health centers
- Educates other departments within Peak Vista through pharmacy/disease-state educational presentations as requested
- Maintains accurate clinic operational records through Microsoft Excel and collates clinic statistical data for reports to Clinical Pharmacy Manager
- Performs regularly scheduled chart audits to identify overlooked patients or appointments and evaluates opportunities for improved clinic or individual performance based on results

Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education/Experience:
Education: Doctor of Pharmacy degree or completion of at least 7 years of experience as a licensed pharmacist.
Work experience: Minimum 1 year experience in clinical pharmacy required.
Additional qualifications: successful completion of an ASHP-accredited post-graduate residency program, current accreditation by Board of Pharmacy Specialties, or CDCES credential required.
Computer Skills: Intermediate - ability to access the intra/internet to manage timecard, review policies and procedures, and read company communications; use e-mail to communicate with co-workers, leadership, and other departments; use and create a variety of templates, complex tables, merges; manage table data, sort and filter merges, and also perform basic work with existing Macros; customize toolbars, import and insert graphs, embed Excel data, and elaborate reports; work with multiple worksheets, filter data, use integrate functions, and manipulate databases; customize templates and the PowerPoint environment, and to make a presentation interactive by using hyperlinks and action buttons. Use electronic health record to review patients, document encounters, create task, order and document labs, order prescriptions
Certificates and Licenses: Required: Registered Pharmacist with a current license in the state of Colorado. Successful completion of an ASHP-accredited residency program, BCACP, CDCES or other accredited BPS accreditations. Meets all Colorado State Board of Pharmacy requirements to engage in Collaborative Drug Therapy Management.
