Compensation (Pay): $117,374.40 to $144,355.71/yearly, based on experience. Plus a $10,000 Sign On Bonus.
*Other compensation may include rural location differentials.

Job Summary: Provide high quality medical care that is culturally appropriate, and prevention-focused, utilizing virtual platforms. Virtual care is the use of synchronous and asynchronous virtual platforms including telehealth platforms, telephone, patient portal, remote patient monitoring, and other electronic platforms Virtual care provides additional tools for improving the health outcomes of patients, reducing access barriers, and providing more affordable care through innovative means. This position is 1.0 virtual care.
Essential Duties and Responsibilities:
- Provides primary care via virtual platforms.
- Focuses on providing access to patients with recent transitions of care (hospital admission, emergency department visit, etc.), care gaps, and meeting other payer requirements via virtual care.
- Maintains accurate records of patient's medical history, prescribed medications, allergies, diagnoses, and progress.
- Advises patients in lifestyle changes to optimize personal health outcomes. Assists patients in managing chronic health conditions.
- Aids in evaluating virtual care platforms (such as patient portal, telehealth platform, remote patient monitoring, etc.) and developing efficient workflows.
- Assists in identifying opportunities for patient outreach, pre/post visit surveys, and number and type of virtual care services provided and follow-up requirements.
- Participates in virtual care PI projects.
- Must reside in Colorado and be willing to work onsite as needed. Position will be 100% onsite initially with option to work remotely over time.

Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Perform all job functions and responsibilities of a quality assurance specialist and provide team education and second level review.
Education/Experience: Our ideal candidate has 2+ years of experience in primary care medicine practicing in an ambulatory setting; 1+ experience with telemedicine; able to utilize a template based EMR (NextGen); has an unrestricted Colorado medical license, as well as an active DEA, is comfortable seeing all ages, and averages 22 patients per 10-hour day. Must have a strong desire to make a difference in the community. Must embrace a team-oriented approach.
Computer Skills: To perform this job successfully, an individual should have knowledge of internet, Microsoft Office (Word, Excel, Outlook, Power Point), other relevant software programs.
Certificates and Licenses: Current CPR (BLS) and Medical License (NP or PA)
