Compensation (Pay): $30.84 to $44.72/hourly, based on experience.
A $5000 sign on bonus will be paid in the following manner: $1250 payout upon 30 days of employment, $1250 payout upon 90 days of employment, $1250 payout upon 6 months of employment, and $1250 payout after 9 months of employment*.
*This bonus only applies to RNs. *Current employees are not eligible for the sign on bonus.
Loyalty Bonus - $5000 upon 1 year of service and then a $1250 payout for every additional 3 months worked. This applies only to FT employees. The amount will be prorated for PT employees. *This bonus only applies to RNs.

Job Summary: The Population Health & Quality Assurance Registered Nurse (RN) is responsible for organizational quality and process improvement initiatives, monitoring clinic performance, program development related to clinical quality and performance measures, and associated education for the organization that align with goals set forth by HCPF, CHPA, AAAHC and other regulatory entities. The Quality Assurance RN works closely with the Quality Oversight RN and Health Informatics Analyst, as well as clinic staff and leadership to identify areas of focus and collaboration.
Essential Duties and Responsibilities:
- Monitor and improve patient outcomes through data analysis, staff education and the implementation of evidence-based practice
- Support preparation efforts for audits and site visits by accrediting organizations
- Evaluate clinical quality measures and provide feedback to clinical staff and clinic leadership regarding their performance
- Monitor organizational performance of Medicare Annual Wellness Visits (AWVs) and provide training to all new clinical staff (providers, MAs and RNs) and ad hoc training as needed
- Assist in grant identification and application for population health programs
- Serve as a project lead on grants that support clinical quality projects
- Collaborate organization-wide with other departments, leadership and clinical staff to identify areas of improvement and interventions to enhance performance and best patient care
- Identify opportunities to implement standing orders, policy and procedures as needed
- Work with providers to assist with closure of care gaps
- Conduct chart reviews as necessary to ensure standards of care and regulatory compliance are being met
- Collaborates with the members of the Population Health department to identify opportunities for improvement and interventions based on the needs of the organization
- Communicate respectfully and effectively to maintain a positive working relationship to achieve desired outcomes
- Collaborates with the Quality Oversight RN to establish areas for process improvement plans
- Collaborates with Health Informatics Analyst to utilize population health data to identify trends, gaps in care, and opportunities for improvement; Assists with New Employee Orientation
- Work with external stakeholders to facilitate partnerships, referrals and quality improvement initiatives
- Collaborate with internal Coding Department to address quality and regulatory concerns
- Must be versatile and adapt to changing job duties and priorities
- Ability to frequently travel to all Peak Vista Community Health Center sites on a regular basis in personal vehicle.

Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education/Experience:
Education: Graduate of an accredited Nursing Program required; BSN preferred.
Experience: 2 years’ nursing experience preferred.
Preferred: Experience with CPT, HCPCS and ICD10 coding.
Certificates and Licenses:
- Current Colorado RN License required; Current CPR (BLS) preferred.
- Applicants must be at least 21 years of age or older to be an eligible driver on behalf of Peak Vista.
- Valid Colorado driver’s license and proof of insurance.
- Must meet and maintain driver criteria as prescribed by Peak Vista’s insurance underwriter.
Other Skills:
- Perform all job functions and responsibilities of the Quality Assurance RN.
- Knowledge of Regulatory Compliance to include governing bodies such as AAAHC, HRSA & HEDIS. Compliance with HIPAA and all applicable rules and regulations.
