Peak Vista Community Health Centers provides exceptional healthcare to people facing access barriers.
We are a private nonprofit dedicated to premier medical, dental and behavioral health services for people of all ages. We provide primary care services to residents of the Pikes Peak Region through 20 outpatient health centers and are accredited by the Accreditation Association for Ambulatory Health Care, Inc. (AAAHC).
Job Summary (Essential Functions):
Performs all routine and emergent nursing functions pertinent to efficient, quality, ambulatory patient care. Optimizes provider schedule utilization by managing patient flow. Monitors medical records and assures patient needs are attended to in a timely manner. Assists providers of the clinic with patient care. Works with outside agencies to assure patient care is being met. Works with referral department to assure patients are being followed up with. Immunization knowledge for all populations. Computer literate and able to achieve competence in current EPM patient management system and current electronic health record system. Able to review and monitor, in conjunction with the charge nurse and clinical team manager, the clinical skills of the ancillary staff. Able to multitask and work as a team leader as needed. Able to assist in and aid in assuring completion of Peak Vista’s quality and performance improvement goals. Assist the clinical team manager in facilitating patient satisfaction and provider productivity goals. Performs other duties as assigned.
Education: Must be a graduate of an accredited Licensed Practical Nurse Program.
Work Experience: Minimum of 1 years of clinical nursing experience required. Ambulatory care/Primary care setting experience preferred.
Cognitive Skills (Language, Math, Reasoning Ability): High language, high reasoning and critical thinking abilities.
Computer Skills: Must be computer literate.
Other Skills: Ability to manage several tasks at one time. Ability to work with a diverse patient population. Bi-lingual (Spanish) preferred
Certificates & Licenses: Current Colorado LPN License and current CPR (BCLS) certification required.
Physical Demands: Able to support/lift at least 50 pounds. Long periods of standing, moving, occasional periods of sitting. Hearing within normal range for telephone use. Must be able to get from one clinic site to related locations.
