Job Summary (Essential Functions)

This Transitional Care Navigator's position is located in the Transitional Care Coordination office at 3207 N. Academy Drive, Suite 1400 in Colorado Springs, Colorado. This position is primarily responsible for facilitating transitional care for patients who have been discharged from the hospital or emergency room. The goal of this position is to promote awareness of the value of patients having an identified Primary Care Provider and/or Patient Centered Medical Home and thus linking patients to primary care for their follow-up and on-going health care needs. This Transitional Care Navigator is responsible for many aspects of patient needs and customer service such as providing patient education and outreach on the continuum of Health Care services offered throughout the community; foster and build relationships with many collaborative partners to include, but not limited to: DHS, EPCDHE, CHP, private medical practices, local hospitals, urgent care and schools to ensure continuity of care to serve as a liaison and link between health and social services to facilitate access to primary healthcare services. They act as a liaison between the patients, providers, programs and resources; screen patients for eligibility for CICP, CHP+ and Medicaid and verify private insurance. Once patients have identified their Primary Care Provider, this Transitional Care Navigator ensures the patients are appropriately attributed; assists in making appointments for patients; provide appropriate referrals into the continuum of services; and perform data entry into the designated Electronic Health Record (EHR).

JOB DUTIES

  1. Receives daily lists of hospital and emergency room patient admissions.
  2. Conducts chart reviews of assigned patients and determines appropriate courses of action.
  3. Assists to ensure appropriate attribution to the Primary Care Providers with which the patient has identified.
  4. Assists the patients in linking with and/or redirecting them back to their Primary Care Providers for follow-up appointments post hospital or Emergency Department visit.
  5. Screens patients for eligibility of CICP, CHP+, Medicaid, and/or verifies private insuranc
  6. Assesses patients for barriers to care and assists them in overcoming the barriers in order to meet their on-going health care needs.
  7. Communicates and collaborates with other service providers and community resources in an effort to ensure holistic, continuity of care for the patient.
  8. Provides patients with appropriate community referrals.
  9. Seeks professional opinions and input from the appropriate providers and/or care teams.
  10. Completes other duties as assigned.

Qualifications

Education: High School Diploma or equivalent required.

Work Experience: 1 year of customer service experience is required. Experience in documenting health records and working in a medical environment is preferred.

Cognitive Skills (Language, Math, Reasoning Ability): Excellent oral and written communication skills, ability to understand complex issues, basic math skills required.

Computer Skills: The ability to type and proficiency with Microsoft Office (i.e. outlook, word and excel) and Internet Explorer is required.

Other Skills: Conflict resolution, attention to detail, knowledge of community resources, confidence, and creativity required. Ability to work both as a team and independently, bi-lingual preferred, healthcare knowledge (e.g. payor sources, etc.) preferred.

Certificates & Licenses: Valid Colorado Driver’s License and up to date proof of insurance required.

Work Environment: Work hours are 8am to 5pm Monday through Friday with a one-hour lunch. Duties will be performed in a typical office environment along-side other navigators.

Physical Demands

Frequent standing and walking, occasional long periods of sitting, moderate bending, moderate energy requirements, and frequent fine motor skills. Looking at computer monitors over an extended period; hearing within normal range; and must be able to lift 20 pounds.
