Job Summary (Essential Functions)
Responsible for clinical coordination of the CCA clinic, including linkage with external partners and patient flow on clinic days. Acts as liaison between patient, specialists, PVCHC providers, and external partners and SCAP. Direct support for patient care providers in the performance of routine and acute clinical functions pertinent to efficient, ambulatory patient care. This would include all paper work necessary for the appointment including and not limited to labs, reports from other agencies, refills on medications, triage, scheduling appointments for medical needs, performing the necessary intake functions for arriving patients, assists in questions, direction of telephone calls and other patient service duties. When necessary this person will function as a float on the floor to further assist Family Health and the needs of the team. Performs other duties as assigned.

Qualifications
Education: Graduate of an accredited Registered Nursing Program required.

Work Experience: Minimum of 1 year clinical nursing experience required. Ambulatory care/primary care setting experience preferred. Triage experience preferred. Experience working with the HIV population preferred.

Cognitive Skills (Language, Math, Reasoning Ability): High language, high reasoning and critical thinking abilities.

Computer Skills: Basic computer knowledge and keyboarding skills.

Other Skills: Ability to remain calm under pressure, manage several projects at one time. Ability to work with a diverse patient population. Bi-lingual (Spanish), sign language preferred

Certificates & Licenses: Current Colorado RN License and current CPR (BCLS) certification required.

Physical Demands
Able to support/lift at least 50 pounds. Long periods of standing, moving, occasional periods of sitting. Hearing within normal range for telephone use. Must be able to get from one clinic site to related locations.
