Job Summary (Essential Functions)

PVCHC is a Federally Qualified Health Center which strives to provide a whole person healthcare model, which includes medical, dental, and integrated behavioral health providers. This model utilizes brief visits for a wide variety of psychosocial issues that present to our centers. Behavioral Health Providers (BHPs) provide consultative assessments and short-term, follow-up interventions, while working with primary care and dental providers, as well as other members of an integrated care team. BHPs collaboratively develop and implement intervention or treatment plans using evidence-based, solution or problem-focused strategies.

Essential Duties/Responsibilities: Provides transparent oversight of the behavioral health operations, in partnership with the Vice President of Behavioral Health assuring accountability, training, and reviewing the following tasks and responsibilities: ·

Team Management

Operational Domain: Work in conjunction with·Vice President of Behavioral Health (as well as other operational VPs & operational directors in the 16 integrated clinics) and provide the operational expertise to integrated care to include the management BH templates, office space issues, deployment of BH floats, BH screening workflows, assist with grant editing/writing and provide oversight and guidance to day to day administration issues as they arise. Team onboarding, development and training: Serves as liaison to UCCS in onboarding students and monitoring schedules
    
Assures team competencies; including but not limited to assuring clinic staff have adequate trainings to conduct behavioral health assessments; Assures certification and trainings are up to date through interdepartmental communication, initiates the hiring. orientation and training processes for new and existing team members (to include providers, clinical and support team members). Ongoing coaching and development focused on team-based care and behavioral health integration into clinics.  Team experience Organizational / Clinical Culture; b-directiopal corporate and team communication; Promotes interdepartmental collaboration, coordination and integration. Mission. Exemplifying the Core Values.  Serve as a liaison and lead communicator between administration and behavioral health providers and their respective clinical leadership. Communication should be presented in a concise, understandable fashion that providessufficient infonnation on the background and reasoning for decisions or initiatives. Seek input and feedback from those affected by changes and share this feedback during Leadership Team Meetings. Develop and facilitate the implementation of process improvement practices to achieve the "quadruple aim"

Patient Management
  • Continual support for the corporate Mission and values through: Maintaining measures centered upon Patient Access; Patient Centered Medical Home (PCMH) focus; Manages and maintains team schedules to support patient access during posted hours of operation.
  • Care Coordination: Contracts / Obligations
    Patient Experience: Works with clinical leadership to identify areas in which behavioral health services can improve to provide better patient access and care. Ensure appropriate visit types and patient groups are occurring in all clinics; i.e. co-visits, flow visits and pain management groups. Management of patient complaints, dismissals, narcotic issues in collaboration with clinical leadership. Patient advocacy/ Exemplary Customer Service.
    Engagement and follow up with patient surveys and feedback: Trending and team coaching/accountability; Patient letters / calls; Comment Cards; and CAHPS
  • Patient Management: Work with VP of BH to: Design, implement, and evaluate population health management programs Program evaluation (i.e. using such metrics as quality metrics for the RAE, NCQA PCMH standards)
  • Community Involvement: Represents PVCHC and communicates its philosophy and mission at community functions. Hosts and coordinates visits for visitors, politicians or other people coming to the clinic. Serves as a liaison to Aspen Point. Cultivates relationships with community resources to expand community access to patients

Quality Management :

  • Maintenance of clinical scope and practices. Operates within Board approved hours of operations. Policies and procedures: Assist as requested in the development of policies and procedures related to operational issues and ensure compliance with all policies and procedures.
  • Patient and Team Safety: Assure safe and well maintained facilities and that signage is clear, up-to-date and professional. Responsibleto coordinate with the PVCHC system the implementation and assessment of adherence to all regulations and compliance standards (OSHA, CLIA, AAHC, Medicare and Medicaid, etc.)
  • Continuous Quality improvement(CQI): Run quality reports, analyze the data and implements a quality improvement process to improve measures. Monitors and seeks to improve measures in compliance with all expected perfonnance measures (to include but not limited): AAAHC, HRSA, CMS, Medicare/Medicaid, UDS Quality Goal, KPI, Internal clinic benchmarks, ACO Quality Goals, and 3rd Party Insurers
Serves as behavioral health operations expert on committees throughout the organization; i.e.pain management committee. Works to implement studies and surveys across the organization.
  • Professional Developmen:t:  Participates in professional development activities to keep current with trends and practices in health care management

  • Electronic Health Record: Maintains current knowledge and skills of all computer programs being used in the health center(s); EHR enhancements for integrated health documentation and data mining.

Finance Management:

  • Productivity and Revenues:

    • Manage provider productivity standards (as established in Annual Encounter Expectation documents). Understand, oversee, and manage the business model of integrated BH care. Develops annual encounter expectations for the BHPs. Give providers their productivity data at least monthly. Explores alternative opportunities to meet standards. Operates within allotted expense budget(s)

  • Adheres to corporate pay structures and staff grading expectations

  • Exemplifies and leads team toward corporate stewardship expectations

  • Manages and supports grant related measures and performance/reporting expectations

  • Complies with Peak Vista Contract management processes and policies
    Performs other duties as assigned.

Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

Suggested Competencies:

Ability to multi-task and to manage several projects at one time and ability to work with a diverse patient population required. Customer service skills, Bi-lingual (Spanish) a plus.

Education/Experience:

Bachelor's degree in Business Administration, Health Administration, Nursing or other health related field strongly preferred. Master's degree is health related field is preferred.

Work Experience: Previous work in healthcare and behavioral health arena, such as practice management or leading a behavioral health clinic required; a minimum of five to ten years experience. Two years of experience in management, leadership and direct supervision.

Language Ability:

Ability to read, analyze. and interpret common scientific and technical journals, financial reports, and legal documents. AbiJity to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to write speeches and articles for publication that conform to prescribed style and format.

 

Math Ability:

Ability to work with mathematical concepts such as probability and statistical inference, and fundamentals of plane and solid geometry and trigonometry. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.

 

Reasoning Ability:

Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.

 

Computer Skills:

To perform this job successfully, an individual should have knowledge of Word Processing software; Internet software and Database software.
