Peak Vista Community Health Centers is a non-profit healthcare organization whose mission is to provide exceptional healthcare to people facing access barriers.  We provide primary care services (medical, dental and behavioral healthcare) through 26 outpatient health centers in El Paso, Teller, Lincoln, Adams, Elbert, and Kit Carson counties.  We deliver care with our strong “Hospitality” culture.  Our organization has over 850 employees and serves 90,000 patients in the Pikes Peak & East Central Plains region.  Peak Vista is accredited by the Accreditation Association for Ambulatory Health Care, Inc. (AAAHC).

This is a part time 20 hour/week position at the new Pain and Wellness Health Center.

Job Summary (Essential Functions)

PVCHC is a Federally Qualified Health Center which strives to provide a whole person healthcare model, which includes medical, and integrated behavioral health providers (BHPs). This biopsychosocial model of health care utilizes BHPs for a wide variety of psychosocial issues and health behavior change for patients presenting to medical providers. BHPs provide joint consultative visits, BH assessments (to include risk or safety assessments), and short-term, follow-up interventions or treatments, while working with other members of the clinical care team. BHPs collaboratively develop with patients and their medical provider’s treatment strategies using evidence-based, solution or problem-focused strategies, and measure progress for their BH treatment with an ultra-brief, patient-directed outcome management system.

Essential Duties/Responsibilities:

  • Provides consultative, psychosocial education, diagnostic assessment, and treatment services to patients with co-occurring chronic pain, mental health and substance use disorders, using and being clinically competent in evidence based practices for chronic pain and co-occurring BH conditions. Must also be skilled and competent in delivering appropriate treatment strategies in individual or group modalities
  • Implement psychometrically sound, patient-centered measures for the assessment and treatment of chronic pain and co-morbid BH conditions that track patient progress in improved functioning or symptom reduction. Assists care team in implementing the appropriate outcome measure of the care team’s effectiveness in improving patient satisfaction and outcomes
  • Applies behavioral principles and other principles of change for prevention, wellness, and health behavior change
  • Assists in the management of the psychosocial aspects of acute and chronic medical conditions, as well as co-managing mental disorders and life stressors
  • Performs assessments of patient’s psychosocial needs and together with the patient, health/dental provider, and other members of the integrated care team assists in developing appropriate care plans
  • Provides consultant services to medical and dental providers based on an integrated Primary Care Behavioral Health Consultation (PCBH) model utilizing co-visits (i.e.warm handoffs)  and short-term, behavioral health 30 minute follow-ups
  • Evaluates and/or revises care plan, as indicated, with appropriate documentation in medical EHR and communicates this to relevant inter-disciplinary team members
  • Knowledge of the common factors and able to form therapeutic relationships with patients through collaborative goal setting. Monitors and tracks outcomes of delivered follow-up services with appropriate assessment/outcome measures
  • Maintains all required EHR documentation on each patient served to ensure coordinated patient services and to help document behavioral health services
  • Maintains organization and patient confidentiality (e.g. HIPPA knowledge)
  • Performs other duties as deemed necessary by supervisors
  • Sustains productivity expectations established by supervisor

Qualifications

Education/Credentials/Formal Training:

  • Licensed Clinical Social Worker (LCSW)
  • CAC II or CAC III a plus. 
  • Currently licensed in the State of Colorado to practice as an LCSW 

Experience:

  • Experience and/or interest in working with medically underserved populations
  • Preferred experience in a medical or health care team environment
  • Experience in working with patients with chronic pain and co-occurring BH conditions.

Physical Demands

Frequent standing and walking, occasional long periods of sitting, moderate bending, moderate energy requirements, and frequent fine motor skills. Looking at computer screen continuously for long periods of time, hearing within normal range. Must be able to lift 25 pounds.
