Job Summary (Essential Functions)

The Dental Program Manager's primary role is to coordinate services for the target patient populations as identified by the grant, and other dental programs.  Have an understanding of how Peak Vista Community Health Centers can help patients with their oral health care.  Will help identify barriers to effective care and assist the patient in identifying and working with community resources that reduce those identified barriers.  In addition to assisting those identified patients, the Program Manager will assist clinical staff by helping identify programs that may benefit patients.  The Dental Program Manager provides one-on-one guidance, support, education, coordinates care with their oral health team; collaborates with dental staff to ensure the most suitable level of care.  Promotes and coordinates oral health services, with cost effective solutions, and contributes to the implementation and  completion of the member's treatment plan.  Manager responsibilities may include, but are not limited to, scheduling and managing appointments, supervising the eligibility process for all dental programs, asking questions and gaining access to services and organizations and facilitating basic oral health education groups.  The Program Manager will oversee patients and act as a liaison t the program in which they are assigned.  Program Manager does not provide clinical care and does not extend substitute the most specialized services of a Dentist or Hygienist.  Performs other duties as assigned.

Qualifications

Education: High School diploma required.  Degree in a healthcare/dental or social service related field is preferred.

Work Experience: Experience in a healthcare setting working with patients, healthcare services, medical/dental groups and community resources are required.  Experience in direct communication with patients in a high volume fast paced environment preferred.

Cognitive Skills: (Language, Math, Reasoning Ability): Excellent verbal and written skills in English and Spanish preferred.

Computer Skills:  Microsoft applications required.

Other Skills:  Ability to flourish independently and in a team management system.  Sensitivity to low income, ethnic minority community.  Excellent organizational skills, excellent tracking and documentation required.  Excellent attendance required.  Excellent listening skills.

Certificates & Licenses:  Valid Colorado driver's license and a dependable and reliable vehicle.

Physical Demands

Frequent standing and walking, occasional long periods of sitting, moderate bending, moderate energy requirements, and frequent fine motor skills.  Looking at computer screen continuously for long periods of time, hearing within normal range. Must be able to life 20 pounds.
