Summary:
Responsible for the management of all functions of the Care Coordination, Clinical Training, Risk Management and Nursing teams. This includes all contractual requirements for the productivity of the department(s), advises medical staff, department heads, and administrators in matters related to these departments.

Essential Duties and Responsibilities include the following:

  • Provides leadership and direction for planning, implementation, and evaluation of activities related to the Nursing staff at all PVCHC sites to include professional development; adherence to clinical guidelines and established protocols; and clinical workflows.
  • Provides leadership and direction for planning, implementation, and evaluation of activities related to centralized clinical support teams, including: Clinical Training team, Care Coordination Team, Transitional Care Team, Lab Services and the Vaccine Coordinator. Represents PVCHC as needed in meetings and collaborates with outside agencies for clinical representation, such as CDPHE, VFC, ACO, RAE, AAAHC, and others to ensure compliance with program requirements and clinical guidelines.
  • Administration of the Risk Management and Infection Control Plans as directed by the Board of Directors and the Chief Medical and Dental Officer to meet the goals of the programs and provide oversight of the investigation of unusual clinical infection exposures or outbreaks, utilizing microbiology laboratory, county/state health departments and if necessary, the Center for Disease Control.
  • Provides leadership, oversight, and strategic vision around the training and improving interdisciplinary team approach for Care Coordination within the organization.
  • Acts as the clinical nursing resource in regards to current and future project management, strategic planning, quality improvement and assurance.
  • Acts as a supportive clinical liaison for accreditation review and provides technical assistance as needed.
  • Monitors productivity goals and facilitates Process Improvement activities as needed.
  • Administration of the Workers Compensation program to include incident reporting, follow up with insurance carrier, employees and medical providers, etc.
  • Supports the partnership with other leadership in the organization to develop and implement protocols to enhance clinical practice and patient services around the Care Coordination workflows.
  • Supports the Electronic Medical Record systems to assure proper documentation of care coordination activities are reportable.
  • Coordinates with Business Development on community Referral networks.
  • Reviews agreements and supports relationships.
  • Reviews Quality information and develops and supports activities for improvements.
  • Collaborates with community partners in completing transitions of care for PVCHC patients.
  • Administration of the Risk Management Plan as directed by the Board of Directors and the President/CEO to meet the goals of the program, to include supporting the Risk Manager in reviewing root cause analysis, conducting investigations and audits (both internal and external), reporting on documented incident reports and developing action plans as needed.
  • Management of employee timecards, PTO requests, evaluations, training and training plans, budgeting and staffing issues
  • Performs other duties as assigned.

Supervision Exercised:
Director of Care Coordination, Director of Nursing & Risk Manager

Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

Education/Experience:

Education:
Bachelors of Science in Nursing required, Masters of Nursing preferred.

Work Experience:

Minimum 5 years of nursing experience required and a minimum of 5 years in a managerial/leadership role required; ambulatory care/primary care setting experience preferred; knowledge of process improvement or care coordination or care management preferred.

Language Ability:
Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to write speeches and articles for publication that conform to prescribed style and format.

Math Ability:
Ability to work with mathematical concepts such as probability and statistical inference, and fundamentals of plane and solid geometry and trigonometry. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.

Reasoning Ability:
Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.

Computer Skills:
To perform this job successfully, an individual should have knowledge of Word Processing software.

Certificates and Licenses:
Current Colorado RN License
