Job Summary (Essential Functions)

The HPV Vaccine Program Coordinator will be responsible for expanding, managing and reporting on the “Peak Vista Prevents HPV” program. The position requires acting as a liaison with the providers and clinical staff for continuity of care, educating both internal staff and parents/young patients about HPV/vaccines, conducting outreach in the community and at our two School Based Health Centers.  Reviews and distributes monthly reports, does chart auditing to ensure HPV status and completion of series.  Participates in and helps coordinate outside audits in all departments that pertain to immunizations.  Maintains and updates all relevant policies and all relevant educational materials.  Participates on evaluation team and provides timely reports to grants department.  Acts as Peak Vista’s point of contact for any HPV and related questions or concerns and will have a complete understanding of the state protocols. HPV RN Coordinator actively participates in local, regional and national coalitions and task forces engaged in HPV and other cancer prevention activities. HPV RN Coordinator sends monthly updates on immunizations to all participating centers as needed, including all VIS information updates, in a timely manner.  Will conduct appropriate literature searches and research as necessary to develop program and its education components.  Works with Immunization Coordinator, Clinical Team Manager, nurses, provider champion and clinical training coordinators to create, maintain and ensure training materials are current and delivered in a timely manner. Position is responsible for HPV training for all staff that will be administering immunizations and maintains the monthly and yearly staff training schedule.  Is available to make patient appointments and as support for all centers as needed.  Responsible for running and coordinating immunization clinics as needed.  Assists with phone calls regarding immunization questions and facilitates follow-up calls to families for completion of series.  Performs other duties as assigned. 


Qualifications

Education:  Graduate of an accredited Nursing Program, "Registered Status" required. Military applicant acceptable with equivalent training.

Work Experience:  Minimum of 2 years of clinical experience required.  Ambulatory care/Primary care setting experience preferred.

Cognitive Skills (Language, Math, Reasoning Ability): Ability to express ideas clearly, effectively and persuasively, both orally and in writing.

Computer Skills:  Ability work within the Microsoft Office Suite; ability/willingness to learn additional job-specific programs.

Other Skills:  Ability to build and maintain strong relationships; Ability to work wth a diverse patient population; Highly organized with an ability to effectively maneuver between multiple responsibilities; Ability to plan, facilitate meetings; Bi-lingual(Spanish speaking, ASL) desirable.

Certificate & Licenses:  Current RN License required.  Valid driver's license and reliable transportation along with valid insurance in accordance with Colorado law required.


Physical Demands

Able to support/lift at least 50 pounds.  Long periods of standing, moving, occasional periods of sitting.  Hearing within normal range for telephone use.  Must be able to get from one clinic site to related locations.
