Description

Peak Vista Community Health Centers is a non-profit healthcare organization whose mission is to provide exceptional healthcare to people facing access barriers.  We provide primary care services (medical, dental and behavioral healthcare) through 25 outpatient health centers in El Paso, Teller, Lincoln, Adams, Elbert, and Kit Carson counties.  We deliver care with our strong “Hospitality” culture.  Our organization has over 600 employees and serves over 10% of the residents in the Pikes Peak region.  Peak Vista is accredited by the Accreditation Association for Ambulatory Health Care, Inc. (AAAHC).

Purpose:  To serve as program support to First Visitor team members, volunteers, and both internal and external partners.

Location:  First Visitor office at International Circle.


Job Summary (Essential Functions)

        Key Responsibilites:

  •  Compiles training materials and provides orientation for First Visitor volunteers
  • Assists with recruiting, screening, background check process, training, coordinating and supervising volunteers and volunteer activities.
  • Assists with developing and implementing volunteer training program curriculum and training calendar, both initial and ongoing training opportunities.
  • Documents all relevant volunteer information and enters/maintains volunteer database or designated tools.
  • Assigns volunteers to client cases, documents all home visits and enters data into appropriate database and/or EPM/EHR, as appropriate, ensuring accuracy of information, notes, and referrals.
  • Provides volunteer support and home visitation or case debrief, as needed by volunteers.
  • Ensures volunteers are supervised at all times while in the First Visitor office.
  • Facilities linkage to and among agencies involved with supporting the family, specifically, the clnics in Peak Vista and the volunteers.
  • Works with Manager to develop and implement a volunteer retention plan, annual volunteer review, and quarterly volunteer satisfaction surveys.
  • Works with Manager's direction to develop partnership opportunities and internal and external events.

Reports to: Manager of Clinical Support Programs

Qualifications

Education:  Minimum high school diploma; prefer Associates Degree in healthcare, psychology or related field.  Knowledge of child development and parent/child relationships.
 
Work Experience: Maturity and 1 year experience in successfully working with children 0-3 and their families, as well as knowledge of existing human services programs and community programs. Experience in data collection and client phone contact is preferred.  Previous volunteer coordination experience preferred.  Minimum 2 years work experience in case management role.
 
Cognitive Skills (Language, Math, Reasoning Ability):  Excellent language, reasoning and critical thinking skills.
 
Computer Skills:  Microsoft Word,  EPM/EHR knowledge preferred.
 
Other Skills:  Knowledge of community resources, works well with diverse groups of people, strong interpersonal skills.  Bi-lingual (Spanish) preferred.
 
Certificates & Licenses: CPR certification.


Physical Demands

Frequent standing and walking, occasional long periods of sitting, moderate bending, moderate energy requirements and fine motor skills. Able to look at computer screen continuously for long periods of time. Hearing within normal range.  Must be able to lift 20 pounds.
