Peak Vista Community Health Centers is a non-profit healthcare organization whose mission is to provide exceptional healthcare to people facing access barriers.  We provide primary care services (medical, dental and behavioral healthcare) through 20 outpatient health centers in El Paso,Teller, Lincoln, Adams, Elbert and Kit Carson counties.  We deliver care with our strong “Hospitality” culture.  Our organization has over 600 employees and serves over 10% of the residents in the Pikes Peak region.  Peak Vista is accredited by the Accreditation Association for Ambulatory Health Care, Inc. (AAAHC).

Job Summary (Essential Functions)

Coordinates and directs the identification and assessment of health education needs and the available resources, oversees a comprehensive health education program, promotes positive health behaviors and works to reduce or eliminate health risk behaviors, documents services provided in a professional manner and maintains confidentiality, participates in the health education evaluation process, communicates effectively both in writing and orally, including documentation, recordkeeping and correspondence.

Supervision Received: Director of Integrated Health Services
Supervision Exercised:  Health Educators and Health Education Assistant

Qualifications

Education: Graduate of a college or university with a Bachelor's degree.

Work Experience: Minimum of 2 years of health related or education experience required; 1 year of staff supervision and/or coordination experience preferred.

Cognitive Skills (Language, Math, Reasoning Ability): High language, high reasoning, critical thinking and problem solving abilities, excellent customer service skills, required. High volume quality work is paramount.  Strong professionalism and confidentiality are also required.

Computer Skills: Must have calendaring and preferably Microsoft Outlook experience.  Experience with Word, Excel, PowerPoint, at minimum; keyboarding skills with accuracy.

Other Skills:Well developed multiple project management; training, meeting minutes required.

Physical Demands

Frequent standing and walking, occasional long periods of sitting, moderate bending, moderate energy requirements and fine motor skills. Looking at computer screen continuously for long periods of time, hearing within normal range.  Must be able to lift 15 pounds.
