Peak Vista Community Health Centers is a non-profit healthcare organization whose mission is to provide exceptional healthcare to people facing access barriers.  We provide primary care services (medical, dental and behavioral healthcare) through 21 outpatient health centers in El Paso and Teller counties.  We deliver care with our strong “Hospitality” culture.  Our organization has over 600 employees and serves over 10% of the residents in the Pikes Peak region.  Peak Vista is accredited by the Accreditation Association for Ambulatory Health Care, Inc. (AAAHC).

Job Summary (Essential Functions)

Provides quality health education in a classroom setting or 1:1 with adults and children. Collaborates with clinic staff to identify patients, assessment of patients health education needs, and determine appropriate teaching methods to meet patient needs. Researches, recommends, and prepares teaching modules for various disease processes such as diabetes, hypertension, managing asthma, healthy weight and wellness. Documents health education services in the patient record utilizing the Electronic Health Record. Establishes care plans based on patient assessment of health education needs. Assists with class scheduling. Supports and assists in completion of Peak Vista’s Quality and Performance Improvement goals.  Instrumental in facilitating patient satisfaction and provider satisfaction for the health care needs of patients and families.  The educator will function as a member of the care coordination team, working positively and collaboratively with a multi-disciplinary team to employ strategies and techniques to manage the patient’s physical, environmental, and psycho-social health issues. Performs other duties as assigned.

Qualifications

Education: Bachelor’s degree in Nutrition/Dietetics, successful completion of Dietetic Internship or equivalent.
 
Work Experience: Minimum 1-2 years clinical experience required. 

Cognitive Skills: (Language, Math, and Reasoning Ability): High language, high reasoning and critical thinking abilities required. Ability to communicate in English, both orally and in writing required. Bilingual in Spanish preferred.
 
Computer Skills: Keyboarding skills and be able to use word processing software at a novice level, electronic health records and other computer based programs.

Licenses and Certifications: American Dietetic Association Registration Registered Dietitian. Colorado driver’s license and insurance as required by state law. CPR required.

Physical Demands

Frequent standing and walking, occasional long periods of sitting, moderate bending, moderate energy requirements, and frequent fine motor skills. Looking at computer screen continuously for long periods of time, hearing within normal range. Must be able to lift 20 pounds.*Successful candidates will complete pre-employment screening; which includes, but is not limited to a Criminal Background check.
