Job Summary (Essential Functions)

Summary: The Clinical Operations Program Coordinator is responsible for supporting the Clinical Operations Leadership Team with regards to a variety of ongoing operational, administrative and reporting functions associated with Clinical Operations’ programs and processes. These programs include clinic specific and company-wide initiative (such as care coordination efforts etc.).

 

Essential Duties and Responsibilities include the following.

  • Manages master documents associated with staffing throughout Clinical Operations. This process includes the collection of information and maintaining accurate lists month to month for use by the Clinical Operations Leadership Team.
  • Supports Clinical Operations Directors with regards to ad hoc requests for meeting minutes and/or other administrative support functions and tasks.
  • Works with other Clinical Ops Coordinators and Assistants in screening, scheduling and coordinating interviews for open positions.
  • Facilitate the coordination across Peak Vista of care coordination process changes, updates and information associated with care coordination within the company.
  • Facilitates and coordinates with the Clinical Operations Training Team in scheduling care coordination trainings across the company.
  • Develop and maintain general policies and procedures for data integration and integrity with regards to centralized care coordination programs, templates and tools utilized throughout the company.
  • Participate in care coordination program / template development meetings with Peak Vista employees and partners associates with the program(s).
  • Maintain working knowledge of all programs/contracts that Peak Vista has agreed to participate in with regards to care management and care coordination (to include RCCO and Private Insurances) - in order to help develop and maintain centralized care coordination workflows and templates so that they meet all expected deliverables.
  • Work with the Clinical Operations Leadership Team to assess and develop processes to support new or changed program requirements as they arise.
  • A thorough knowledge of NextGen EHR and PM functionality will be essential to success in this role. Work directly alongside the Training Manager and other training specialist to help develop, train and troubleshoot all aspects of the care coordination templates and workflows.
  • Works in conjunction with other departments and Clinical Operations team members in collecting, normalizing and maintaining data associated with business partners, and referral sources that receive Peak Vista patients. This list to be utilized to support the BI and IT teams in updating pick lists and other functions associated with Care Coordination reporting and template functionality.
  • Receives and oversees the dispersing of care coordination reports (to include referral reports and others). In addition, as questions concerns or other issues arise, the coordinator will help to troubleshoot and resolve these issues.
  • Employee demonstrates knowledge of and adherence to the Compliance Plan and conflict of interest requirements. The employee enhances the effectiveness, efficiency and productivity of the department by contributing to, and participating in, departmental performance improve goals.
  • Cross-trained to support the other Clinical Operations Coordinators and Assistants as needed.
  • Willing to travel and support various administrative functions in clinics throughout Peak Vista as needed by the Clinical Leadership Team.
  • Other duties may be assigned.

Qualifications

Education: 2 years or more of higher education preferred. (Work experience may be substituted; 2 years' experience in health care arena for every one year of higher education requirement.)

Work Experience:  Minimum 2 years of administrative or clerical experience in a healthcare or social service setting is strongly preferred. 

Cognitive Skills (Language, Math, Reasoning Ability):  High language, reasoning and critical thinking abilities, strong decision making and organizational skills.  Spanish bilingual preferred.

Computer Skills:  Moderate computer and keyboarding skills required.

Other Skills:  Ability to work with a diverse group of people, handle multiple issues that are time sensitive.

Certificates & Licenses:  Any healthcare certificate or license preferred.


Physical Demands

Frequent standing and walking, occasional long periods of sitting, moderate bending, moderate energy requirements, and frequent fine motor skills. Looking at computer screen continuously for long periods of time, hearing within normal range.  Must be able to lift 40 pounds.
