Job Summary (Essential Functions)

Responsible for supervision of patient navigation  in conjunction with providing enhanced access to care and services, reducing barriers to care, and increased patient satisfaction. Connects with community partners and facilitates information back to Patient Navigation staff.  Has knowledge of Patient Navigation functions and community resources for Medicaid / Medicare/CICP populations.  Has knowledge of Disease Registry management. Provides support and insight to the Manager of Care Coordination in relation to service provided to our patients referred for Care Coordination resources. Supports many internal departments by providing quality customer service. Other duties as assigned.

Supervision Exercised: Patient Navigators

Qualifications

Education: High School Diploma required. Bachelor’s Degree in Human Services, Social Work or related Healthcare Field preferred or equivalent work experience. Medical Assistant certificate or equivalent military experience preferred.

Work Experience: Minimum 2 years of experience in providing community resource referrals required.  Previous work in a health care setting preferred. Supervisory experience preferred.
 
Cognitive Skills (Language, Math, Reasoning Ability): Excellent language, reasoning, critical thinking, and decision making skills.  Demonstrates strong leadership and organization skills.
 
Computer Skills:  Working knowledge of Microsoft Word, Excel, PowerPoint, and Access software; Ability to perform internet research; Ability to use an Electronic Health Record.
 
Other Skills:  Self-directed and able to organize and manage multiple tasks/ projects simultaneously.  Ability to maintain strict confidentiality.  Ability to promote and build teamwork and multidisciplinary care concept.
 
Certification: Patient Navigation Certification within 90 days of hire.
