Peak Vista Community Health Centers provides exceptional healthcare to people facing access barriers.

We are a private nonprofit dedicated to premier medical, dental and behavioral health services for people of all ages. We provide primary care services to residents of the Pikes Peak Region through 20 outpatient health centers and are accredited by the Accreditation Association for Ambulatory Health Care, Inc. (AAAHC).


Job Summary (Essential Functions)


Provides health education including diabetes education and counseling for patients to enable them to achieve and maintain good diabetic control and overall health. Provides individualized medical nutrition therapy and dietary counseling that is based on assessment and goals for a variety of chronic diseases that can be improved with nutritional and dietary changes. Works with individuals and families to assess nutritional needs and develop dietary management and educational plan and assists patients in implementing dietary management goals. Responsibilities include assessment and evaluation, case management, self-management education, and assistance in coping with the psychological impact of chronic illnesses. The dietician will utilize a Glucometer to train patients on the importance of diabetes self-management. Ability for redevelopment of educational materials and assessment tools is necessary for the dietician to meet the needs of the patient population. The dietician will participate in continuing education classes to remain current with the standards of practice. The dietician will travel to other off-site locations to provide group and individual education to patients regarding their chronic disease management and nutrition therapy. This position may involve driving to client’s homes for assessment and follow-up.  A registered dietitian with a certification in diabetes education (CDE) is preferred. A certified dietician also manages diabetic medications and makes changes as needed to achieve desired outcome. Performs other duties as assigned.

Qualifications

Education: Must be a graduate of an accredited dietician program. Current National certification as a diabetes educator (CDE) or currently working towards certification required.

Work Experience: Two years in a Diabetes Education program, experience in nutrition counseling, teaching required.

Cognitive Skills (Language, Math, Reasoning Ability): High language, high reasoning and critical thinking abilities required.

Computer Skills: Must be computer literate.

Other Skills: Manage several projects/tasks at one time; work with a diverse group of people, work under pressure, excellent time management skills, knowledgeable of community resources. Bilingual (English/Spanish) preferred.

Certificates & Licenses: Registered Dietitian, CDE required.


Physical Demands

Frequent standing and walking, occasional long periods of sitting, moderate bending, moderate energy requirements, and frequent fine motor skills. Looking at computer screen continuously for long periods of time, hearing within normal range. Must be able to lift 20 pounds.
