Description

Peak Vista Community Health Centers is a non-profit healthcare organization whose mission is to provide exceptional healthcare to people facing access barriers.  We provide primary care services (medical, dental and behavioral healthcare) through 25 outpatient health centers in El Paso, Teller, Lincoln, Adams, Elbert, and Kit Carson counties.  We deliver care with our strong “Hospitality” culture.  Our organization has over 600 employees and serves over 10% of the residents in the Pikes Peak region.  Peak Vista is accredited by the Accreditation Association for Ambulatory Health Care, Inc. (AAAHC).

Job Summary (Essential Functions)

  • Recruits, screens, facilitates background checks, trains, coordinates and supervises volunteers and volunteer activities.
  • Develops and implements volunteer training program curriculum and training calendar, both initial and ongoing training opportunities.
  • Documents all relevant volunteer information and enters/maintains volunteer database or designated tools.
  • Assigns volunteers to client cases, documents all home visits and enters data into appropriate database and/or EPM/EHR, as appropriate, ensuring accuracy of information, notes, and referrals.
  • Ensures volunteers are supervised at all times while in the First Visitor office.
  • Provides volunteer support and home visitation or case debrief, as needed by volunteers.
  • Provides all relevant data for internal and financial reports and grant data collection, ensuring that all internal and financial reporting and grant-related data is tracked, timely, and reported appropriately.
  • Works with Manager to develop and implement a volunteer retention plan, annual volunteer review, and quarterly volunteer satisfaction surveys.
  • Works with Manager’s direction to develop partnership opportunities and internal and external events.
  • Utilizes research data on volunteer social mapping, high density areas, and volunteer social needs.
  • Consults with First Visitor team members on referrals and volunteer visits and keeps Manager and other team members informed about First Visitor program updates.
  • Maintains and updates community resource guide.
  • Participates in appropriate community groups, as assigned by Manager.
  • Manages internal program records and office supplies.
  • Follows all relevant policies and procedures, observes all compliance requirements, and attends trainings as requested by Manager.
  • Performs other duties as assigned.

Qualifications

Education:  BA in human services-related field or exceptional, commensurate experience.
 
Work Experience: Minimum 2 years work experience in Case Management.  Minimum 2 years work experience in Volunteer Management.
 
Cognitive Skills (Language, Math, Reasoning Ability):  Excellent language, reasoning and critical thinking skills.
 
Computer Skills:  Microsoft Word, Excel, Bright Beginnings database, EPM/EHR knowledge.
 
Other Skills:  Ability to manage multiple tasks at a time; adept at working with diverse groups; excellent time management skills; thorough knowledge of community resources; strong interpersonal skills.  Bi-lingual (Spanish) preferred.
 
Certificates & Licenses: CPR certification.


Physical Demands

Frequent standing and walking, occasional long periods of sitting, moderate bending, moderate energy requirements and fine motor skills. Able to look at computer screen continuously for long periods of time. Hearing within normal range.  Must be able to lift 20 pounds.
