Peak Vista Community Health Centers is a non-profit healthcare organization whose mission is to provide exceptional healthcare to people facing access barriers.  We provide primary care services (medical, dental and behavioral healthcare) through 25 outpatient health centers in El Paso, Teller, Lincoln, Adams, Elbert, and Kit Carson counties.  We deliver care with our strong “Hospitality” culture.  Our organization has over 600 employees and serves over 10% of the residents in the Pikes Peak region.  Peak Vista is accredited by the Accreditation Association for Ambulatory Health Care, Inc. (AAAHC).

Job Summary (Essential Functions)
This position is responsible for the supervision and development of the Community Assessment department.  The primary area of focus will include the creation, maintenance and implementation of a community-wide assessment strategy encompassing direct patient care, ancillary and enabling services and other critical psycho-social support networks that encompass the depth and breadth of 'health equityh.'  The primary responsibility of this position is to turn quantitative data into accessible and relevant information that is actionable for Peak Vista as well as larger community purposes.  The Director of Community Assessment is responsible for fostering an environment of continuous improvement and efficiency.  The position reports to the Vie President of Administrative Services, and exercises direct oversight of the Epidemiologist.

  •     Determine health issues, based on both qualitative and quantitative data, affecting populations and sub-populations within our communities.
  •     Effectively communicate the relevance of health data to a variety of audiences.
  •     Strengthen and support community efforts to coalesce and form strategic alliances to address Health disparities and economic inequities.
  •     Serve as a catalyst and trusted convener for community-based participatory assessment/focus groups with communities, neighborhoods and residents
  •     Assure provision of reliable assessment data to programs within the community for planning and education purposes
  •     Present assessment information to internal and external audiences tailored to the venue and understanding level of the target audiences.
  •     Assure development, implementation and dissemination of Population Health analysis and reporting as it relates to PVCHC and identified community partners
  •     Organize and support community research, understanding, development and implementation of strategies to advance health equity.
  •     Collaborate with Clinical and Administrative Departments to build support for community-based activities/programming.
  •     Develop, recommend, and implement departmental goals as related to the corporate strategic plan
  •     Assure the development of PVCHC Needs Assessment documents as necessary
  •     Foster cohesive 'hospitality' orientation among departmental staff, recognizing that Community Assessment supports the various departments within the agency in attaining the organizational mission.
  •     Recommend budgets for program department to the Vice President
  •     Monitor budget to assure department is within limits
  •     Participate in various teams as necessary
  •     Provide direct oversight of Epidemiologist, including but not limited to: hiring, firing, personnel action and oversight of departmental progress
  •     Perform related and 'other duties' as assigned.


Qualifications

  •     Minimum of 3 years experience developing and maintaining community-based relationships
  •     Bachelor's degree in public health, community health, communications, psychology or related field
  •     Strong research, analysis and critical thinkings skills
  •     Demonstrated ability to build and maintain strong relationships


Physical Demands

Frequent standing and walking, occasional long periods of sitting, moderate bending, moderate energy requirements, and frequent fine motor skills. Looking at computer screen continuously for long periods of time, hearing within normal range. Must be able to lift 20 pounds.
