Job Summary (Essential Functions)

Responsible for day-to-day management of the centralized care coordination department.  This would include incoming referral management, disease management and case management. Has an understanding of case managers and resource navigators’ job functions. Performs telephonic assessments on referred patients and recommends levels of treatment as well as case assignment. Organizes and maintains caseloads for centralized team members. Leads team in assuring completion of Quality and Performance Improvement goals.  Conducts regular team meetings and meets all deadlines for reporting on assigned programs.  Coordinates communication with members of the health care team and educates the care team about treatment options, community resources and psychosocial concern to aid in the treatment of the patient; supports Case Management team on home visits as needed. Performs other duties as assigned.

Supervision Exercised: Care Managers, RN Care Managers, Resource Navigators

Qualifications

Education: Bachelor’s degree in Nursing preferred.Healthcare Management, Organizational Leadership and/or Public and Non-Profit Management preferred; or Master’s degree in Social Work, Nursing or other related field.
 
Work Experience:: Minimum of 1-3 years nursing experience required with a minimum of 1 years in a nursing leadership preferred; ambulatory care/primary care setting experience preferred; knowledge of process improvement  and previous work in case management program or other social-work or home visitation oriented program preferred. 
 
Cognitive Skills (Language, Math, Reasoning Ability): High language, high reasoning, critical thinking abilities, strong decision making and delegation skills; Able to demonstrate strong leadership and organization skills.
 
Computer Skills: Working knowledge of Microsoft Word, Excel, PowerPoint, and Access software; Ability to perform research on the internet; Ability to use an electronic health record.
 
Other Skills: Ability to work, manage multiple projects, ability to manage several tasks at one time, ability to work with a diverse patient population
 
Certificates and Licenses: Certified Case Manager (CCM) preferred. Current Unemcumbered Colorado RN license required

Physical Demands

Able to support/lift at least 25 pounds. Long periods of standing, moving, occasional periods of sitting. Hearing within normal range for telephone use. Must be able to get from one clinic site to related locations. Must be able to drive to patient’s homes for visitation as needed.
