Job Summary (Essential Functions)

Performs all routine and emergent nursing functions pertinent to efficient, ambulatory patient care, including telephone and face-to-face triage. Optimizes provider schedule utilization by managing patient flow. Monitors electronic medical records and assures patients' needs are attended to in a timely manner. Assists providers of the clinic with patient care. Works with outside agencies to assure patient care is being met. Computer literate and able to achieve competence in current IDX patient management systems, CIIS, outside hospital records and PVCHC's current electronic health record system. Able to review and monitor, in conjunction with the Clinical Team Manager, the clinical skills of the ancillary staff. Responsible for monthly competencies for support staff in conjunction with the training coordinator. Able to multitask and work as a team leader as needed. Is able to back up the Clinical Team Manager in their time of absence to include: staffing of center, monitoring schedules, patient complaints, time card changes and any other need that should arise in their time of absence. Able to assist in and aid in assuring completion of Peak Vista's Quality and Performance Improvement goals. Assists the Clinical Team Manager in facilitating patient satisfaction and provider productivity goals. The RN will be key is assisting the case management with specific patient populations. This person will be familiar with all payor sources accepted by PVCHC and external providers. The RN will work with the providers to assure that all care of the patient is assumed and addressed. Performs other duties as assigned.


Qualifications

Education: Graduate of an accredited Registered Nursing Program.

Work Experience: Minimum of 1 year clinical nursing experience required. Ambulatory care and Primary care setting experience preferred. Triage experience preferred.

Computer Skills: Must be computer literate.

Other Skills: Ability to manage several tasks at one time. Ability to work with a diverse patient population. Bi-lingual (Spanish) preferred.

Certificates & Licenses: Current Colorado RN License. Current CPR (BCLS
) certification required. 

Physical Demands

Able to support/lift at least 50 pounds. Long periods of standing, moving, occasional periods of sitting. Hearing within normal range for telephone use. Must be able to get from one clinic site to related locations.
