Job Summary (Essential Functions)

This position is responsible for the monitoring and oversight of contracts and other agreements related to Population Health Management currently in place with Peak Vista CHC. This includes, but is not limited to the Regional Coordinated Care Organizations (RCCOs), Accountable Care Organizations (ACOs) and other public and/or private entities contracting for patient care. The primary areas of responsibility will be the monitoring of program deliverables and the reporting of these measures and related performance activities to both internal and external stakeholders. Position will work closely with Nursing and Operations staff to ensure complete and timely identification and communication of contractual agreements and any associated implementation challenges. Position will work closely with the Director of Community Assessment to strategically develop programming opportunities based on identified gaps in community services. This position is also responsible for the oversight of Clinical Support Services and Medical Application Site (Medicaid Determination) functions. The Director of Community Programs is responsible for fostering an environment of continuous improvement and efficiency. 
 
Oversight Responsibilities
This position receives oversight from the Vice President of Community Programs
This position exercises oversight of the Manager of Clinical Support Services, the Medicaid Application Site Manager, and the HPV Program Coordinator.
 
Responsibilities/Essential Functions:

  • Develop, refine and implement reporting processes to meet relevant contractual requirements
  • Effectively communicate identified outcomes measures and key performance measures, as represented in relevant contractual agreements, to both internal and external stakeholders
  • Assure compliance with outcomes reporting requirements of contracted funders
  • Assist in negotiating appropriate measures and contractual responsibilities with key payors
  • Develop, recommend, and implement departmental goals as related to the corporate strategic plan
  • Provide direct supervision of the managers of Clinical Support Services and the Medicaid Site , including but not limited to: hiring, firing, personnel action and oversight of departmental progress
  • Support Program staff in community relationships
  • Foster cohesive ‘hospitality’ orientation among departmental staff, recognizing that Community Programs supports Clinical Operations in attaining the organizational mission
  • Recommend budgets for program department to the Vice President
  • Monitor budgets to assure departments are within limits
  • Participate in various teams as necessary
  • Perform related and ‘other duties’ as assigned

Experience Required/Preferred:

  • Ability to express ideas clearly, effectively, and persuasively, both orally and in writing
  • Demonstrated ability to build and maintain strong relationships both internally and externally
  • Minimum of 2 years’ experience monitoring and administering community-based programs
  • Minimum of 3 years’ experience with personnel management
  • Bachelor’s degree in business, nursing, public health

Physical Demands

Frequent standing and walking, occasional long periods of sitting, moderate bending, moderate energy requirements, and frequent fine motor skills. Looking at computer screen continuously for long periods of time, hearing within normal range. Must be able to lift 20 pounds.
