(AAAHC).

Job Summary (Essential Functions)

Responsible for day-to-day management of care coordination department to include referral management and management of the Health Education team. Responsible for identifying and responding to the various needs of our patients in conjunction with providing exceptional customer service. Provides support and insight to the Director of Care Coordination in relation to services provided to our internal and external customers. Responsible for referral department and ensuring the supervisor participates in employee training, quality assurance and coordinating/scheduling needed coverage. Ensures the supervisor has the ability to demonstrate a thorough knowledge of all payer sources accepted by PVCHC and external providers. Identifies the need for new memorandums of understanding (MOU) and directs the supervisor in establishing these while monitoring the appropriateness and effectiveness of existing agreements. Participates in the development of new and the revision of department policies and procedures. Responsible for the creation and workflow of provider schedules for the Health Educators. Triages Health Education referrals and assigns patients to the appropriate team member for quality care. Works with outside agencies to assure patient care is being met. Is able to back up the Health Education team to include: staffing of center, monitoring schedules, patient complaints, and time card changes. Able to assist in and aid in assuring completion of Peak Vista's Quality and Performance Improvement goals. Routinely monitors budget for assigned programs and aids in development and provides input regarding budget as assigned. Assists the Director in facilitating patient satisfaction and provider productivity goals; other duties as assigned.

Job Duties

  • Ensures that staff follow the care coordination process and recommends Performance Improvement (PI) measures where needed
  • Coordinates care coordination team including referrals department and health education department and directs them to most appropriate resources for patient/family and assures appropriate delivery of services
  • Triages referrals and refers patients to treatment as well as case assignment
  • Ensures that referrals are coordinated appropriately and in a timely manner
  • Carries out supervisory responsibilities in accordance with Peak Vista's policies and applicable laws.
  • Responsibilities include:
  • participates in hiring and training new employees and develops annual training plans;
  • planning, assigning and directing work;
  • appraising performances; rewarding and disciplining employees;
  • addressing complaints and resolving problems;
  • approves timecards, authorized PTO requests assuring appropriate program coverage
  • Exercises judgment in administrative matters consistent with agency policies
  • Ability to resolve conflict with staff, providers, patients and families as needed
  • Suggest changes to electronic health record templates as needed to director
  • Facilitates ad hoc meetings with interdisciplinary teams for case management purposes
  • Attends required administrative meetings, participates in discussions
  • Represents Peak Vista at community meetings as assigned
  • Completes Budget Variance reports and routinely monitors budget for assigned programs and aids in development and provides input regarding budget as assigned
  • Assists director in assuring program deliverables and reporting requirements meet established timelines
  • Maintains a professional demeanor at all times
  • Assures staff maintains professional licensure and certification as required by position

Qualifications

Education: Graduate of an accredited Registered Nursing Program and management experience required.

Work Experience: Minimum of 2-4 years in management of diverse multifunctional teams, previous work in a case management program such as Nurse Family Partnership or other social-work or home visitation oriented nursing, previous work experience in the health care arena desired, minimum of 1 years of clinical nursing experience required, ambulatory care or primary care setting experience preferred and triage experience preferred.

Cognitive Skills (Language, Math, Reasoning Ability): High language, high reasoning, critical thinking abilities, strong decision making and delegation skills. Able to demonstrate strong leadership and organizational skills.

Computer Skills: Working knowledge of Microsoft Word, Excel, PowerPoint, and Access software. Ability to perform research on the internet. Ability to use an electronic health record.

Other Skills: Ability to work, manage multiple projects, ability to manage several tasks at one time, ability to work with a diverse patient population; understanding of Physician Consultation, Radiology Referrals, CICP, Medicare, Medicaid, Medicaid PCP, Medicaid Colorado Access, CHP+, commercial insurances and undocumented patients preferred. Basic knowledge of ICD-9 and CPT-4 codes preferred. Understand the processes and needs of community specialty provider offices. Understand the process of obtaining insurance authorization, ordering/faxing to local hospitals and therapies

Certificates & Licenses: Current Colorado RN License.

Physical Demands

Frequent standing and walking, occasional long periods of sitting, moderate bending, moderate energy requirements and fine motor skills. Looking at computer screen continuously for long periods of time, hearing within normal range. Able to travel from clinic to clinic as required and make home visits as needed. Must be able to lift 20 pounds.
