Job Summary (Essential Functions)

Helps provide access to the right care, at the right time, in the right order, in the right setting. Responsible for assisting high-risk patients in achieving maximum wellness through a collaborative, interdisciplinary process involving the patient, family and/or care-givers as well as providers and other clinical support staff.  Using nursing assessment skills to determine the need for case management services and serving as a liaison between patients, their families and healthcare providers, assists in the development of care plans which will include preventative, therapeutic, rehabilitative, psychosocial, and clinical interventions to ensure continuity of care toward the goal of optimal wellness. Utilizes Motivational Interviewing to elicit change talk with patients. Assesses and works on the patient’s readiness to change, the importance of change and confidence in ability to change. Provides or arranges patient education and builds therapeutic relationships with both patients and families. Regularly evaluates these plans of care and update them on an as-needed basis. Must be able to integrate nursing case management with social-work case management and work collaboratively with the multidisciplinary case management team members to support patient goals. This position may involve driving to patient’s homes for assessment and follow-up. Scope of authority includes: providing resource coordination, follow up, patient education, and self-management education services to patients and to serve as a consultant to medical staff; requesting support from professional staff when required.

Performs other duties as assigned.

Qualifications

Education: Graduate of an accredited Registered Nursing Program required. BSN preferred.

Work Experience: Two years experience in case management and extensive knowledge of community resources required. Clinical nursing experience is an asset

Cognitive Skills (Language, Math, Reasoning Ability): Excellent language, reasoning and thinking abilities

Computer Skills: Word, Excel, Electronic Health Record

Other Skills: Knowledgeable of community resources, sensitivity to low income and diverse groups of people, ability to work with families from a strengths-based model and strong interpersonal skills. Self-motivated. Valid driver’s license and insured automobile required. Must be able to work a flexible schedule.

Certifications & Licenses: Current Colorado RN License. Current CPR (BCLS) certification 


Physical Demands

Frequent standing and walking, occasional long periods of sitting, moderate bending, moderate energy requirements and fine motor skills. Looking at computer screen continuously for long periods of time, hearing within normal range. Able to travel from clinic to clinic as required and make home visits as needed. Must be able to lift 20 pounds.
