Job Summary (Essential Functions)
The Perinatal Nurse Coordinator does an assessment of all potential prenatal patients prior to the patient establishing prenatal care at the Women’s Health Clinic. Implements, evaluates and adjusts care of the patient after comprehensive health history with regards to psychological, socioeconomic, cultural and environmental factors. Needs are identified and services are provided to meet them, either by qualified staff of the facility, or by referral, based on established procedure to an appropriate agency. Other duties include: assists with telephone/face-to-face triage, perform nurse visits as needed and enters data into the Prenatal Database. Serves as the charge nurse for the Women's Health team when assigned by the clinical nurse manager. Performs other duties as assigned

Qualifications
Education: Graduate of an accredited Registered Nursing Program. BSN preferred

Work Experience: Minimum of 2 years of clinical nursing experience required. OB/GYN experience preferred. Case management experience desired

Cognitive Skills (Language, Math, Reasoning Ability): High language, high reasoning and critical thinking abilities

Computer Skills: Must be computer literate-will need to learn EPM, EHR, LabCorp program, Physician Link and Prenatal database, keyboarding skills

Other Skills: Ability to manage several projects at one time. Ability to work with a diverse patient population. Bi-lingual (Spanish) preferred; sign language

Physical Demands
Able to support/lift at least 50 pounds. Long periods of standing, moving, occasional periods of sitting. Hearing within normal range for telephone use. Must be able to get from one clinic site to related locations.
