Peak Vista Community Health Centers is a non-profit healthcare organization whose mission is to provide exceptional healthcare to people facing access barriers.  We provide primary care services (medical, dental and behavioral healthcare) through 25 outpatient health centers in El Paso, Teller, Lincoln, Adams, Elbert, and Kit Carson counties.  We deliver care with our strong “Hospitality” culture.  Our organization has over 600 employees and serves over 10% of the residents in the Pikes Peak region.  Peak Vista is accredited by the Accreditation Association for Ambulatory Health Care, Inc. (AAAHC).

Job Summary (Essential Functions)
Supports Peak Vista’s Quality Assurance Program, Performance Improvement Program and Risk Management Program. Provides direction and management for Lab Manager, Risk Management Specialist, and Infection  Prevention RN in accordance with Peak Vista management expectations. Audits health records for proper documentation ensuring compliance with quality standards; provides recommendations for identified gaps. Assesses the quality of services to ensure they meet standards of care established by third-party payors and accreditation bodies. Facilitates the Performance Improvement Review Team and coordinates Performance Improvement activities to ensure strategic leadership and best practices. Identifies systemic and process-related deficiencies and provides recommendations in conjunction with training department. Acts as a supportive clinical liaison for accreditation review and provides technical assistance as needed. Represents PVCHC as needed in meetings with outside agencies. Responsible for risk identification, risk evaluation and coordination of corrective action implementation related to risk issues. Coordinates risk programs. Prepares Risk reports for the Quality Assurance Committee and the PVCHC Board of Directors. Supports Infection Prevention activities, surveillance, and reporting as established by the AAAHC, and the local and state health departments and the CDC. Performs other duties as assigned.


Qualifications
Education: Graduation from an accredited BSN program or a RN with a Bachelor in related field.

Work Minimum of 3 years of nursing experience required; ambulatory care/primary care setting experience preferred. 3 years of experience in process improvement in a clinical setting required. Supervisory experience preferred.

Computer Skills: Excellent language, reasoning, and critical thinking abilities required. Calm under pressure, excellent conflict management skills required. Highly objective, seeks to obtain and understand all pertinent facts prior to reaching conclusions and able to handle multiple deadlines simultaneously.

Other Skills: Able to manage several projects at one time and ability to work with a diverse patient population and leadership team is required.


Certificates & Licenses: Current Colorado RN License required. Colorado Driver's License and insurance as required by state law.

Physical Demands
Frequent standing and walking, occasional long periods of sitting, moderate bending, moderate energy requirements, and frequent fine motor skills. Looking at computer screen continuously for long periods of time, hearing within normal range. Must be able to lift 20 pounds.
