Peak Vista Community Health Centers is a non-profit healthcare organization whose mission is to provide exceptional healthcare to people facing access barriers.  We provide primary care services (medical, dental and behavioral healthcare) through 25 outpatient health centers in El Paso, Teller, Lincoln, Adams, Elbert, and Kit Carson counties.  We deliver care with our strong “Hospitality” culture.  Our organization has over 600 employees and serves over 10% of the residents in the Pikes Peak region.  Peak Vista is accredited by the Accreditation Association for Ambulatory Health Care, Inc. (AAAHC).

Job Summary (Essential Functions)

Navigate and integrate patients through the healthcare system. Collaborate with provider staff to assess client needs. Have knowledge of and determine eligibility for programs offered to patients. Refer patient to community resources to assist in services such as financial assistance, language barriers, transportation and other barriers that occur. Collaborate with provider to ensure optimal patient success with treatment plans. Provides self management support by creating action plans that are patient specific.  Provides integration, referrals, and coordination to all services.  Works with providers to customize services to best meet patient's needs within parameters of available services. Identifies issues and makes recommendations to the patient to facilitate and improve healthier outcomes.  Maintains regular and accurate charting in patients chart.  Coordinates, reviews and maintains daily logs for reporting purposes. Attends regular Huddle meetings for all departments to ensure and maintain integration and communication.  Motivate, educate and support patients and staff.  Provides ongoing phone contact with patients.

Qualifications

High school degree or equivalency.  Maintain clinical licensure as LPN or RN.  Knowledge of computers and electronic medical record systems.  Ability to interact effectively and in a supportive manner with persons of all backgrounds.  Excellent communication skills.  Written and verbal fluency in English and some Spanish.  Excellent organization skills and works accurately, detailed and efficiently and able to multi-task assignments in a timely fashion.  Ability to take blood pressures and operate blood testing machines.

Physical Demands

Ability to sit, walk and/or stand for prolonged periods. Requires sufficient near vision to read documents and computer screens.  Willing to travel to satellite clinics and outreach services.
