Job Summary: The primary role of the Patient Panel Specialist is to oversee processes associated with transferring Primary Care Providers (PCP) within Peak Vista. This is to include: Supporting the Enrollment Services Department, Customer Service Center and Clinical Operations in implementing processes that efficiently and effectively re-empanel new patients with Peak Vista providers; serving as the primary point of contact for the patient transfer process; managing all relevant patient transfer reports and databases.
 
Essential Duties and Responsibilities include the following. 

  • Will use standardized script for patient transfer expectations
  • Receives and process transfer request in a timely manner.
  • Tracks transfer requests for accuracy, timeliness and patient experience.
  • Supports clinical and administrative regarding troubleshooting and correcting any issues with the patient transfer process.
  • Provides feedback regarding the patient transfer process to Peak Vista Leadership.
  • Provides data and reports for leadership for decision making purposes.
  • Works patient transfer OMB daily
  • Scan patient transfer forms into EHR
  • Supports questions from patients and staff regarding processing issues
  • Updates patient transfer tracker daily
  • Communicates appropriately with Clinical Leadership regarding transfer requests and patient needs.
  • Participates in the provider panel management team.
  • Other duties as assigned. 


Qualifications

Education:

Applicant must have a GED or equivalent certification. Experience in health care arena highly preferred.

Work Experience:

Administrative or clerical experience in a healthcare or social service setting is strongly preferred.

Language Ability:

Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.

Math Ability:

Ability to work with mathematical concepts such as fractions, percentages, ratios, and proportions to practical situations

Reasoning Ability:

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret and implement a variety of instructions furnished in written, oral, diagram, or schedule form.

Computer Skills:

Due to the administrative and clerical functions associated with this job, a thorough understanding of word processing, spreadsheet manipulation and processing and exceptional customer service are all essential functions of this role.

Other Skills: Ability to flourish independently and in a team management system. Excellent organizational, listening, de-escalation, tracking and documentation skills required. Excellent attendance required. Bilingual (Spanish) desired.


Physical Demands

The employee must occasionally lift and/or move up to 50 pounds. While performing the duties of this job, the employee is frequently required to stand; walk and sit.
