Job Summary (Essential Functions)

The Resource Navigator’s primary role is to assist patients with identified disease states (Diabetes, Cardiovascular Disease, Depression or Asthma) in understanding their disease, identifying barriers to effective care, and assisting the patient in identifying and working with community resources that reduce those identified barriers. In addition to assisting those identified patients, the Resource Navigator will assist clinical staff by helping identify patients that may benefit from Navigation by working off of disease registries and facilitating follow-up calls with the patient and identified resources to which the patient was referred. The Resource Navigator provides one-on-one guidance, support, education, coordinates care with the case management team, and other assistance to patients as they move through the healthcare continuum; collaborates with clinical staff, Emergency Diversion team via a case manager and community providers to ensure smooth transition to the most suitable level of care. Promotes patient education and self-care management in order to reduce hospital readmission rates; through interdisciplinary collaboration with the case management team to achieve measurable patient outcomes. Promotes and coordinates healthcare services, with cost effective solutions, and contributes to the implementation of the member’s care plan. Navigator responsibilities may include, but are not limited to, scheduling and managing appointments, asking questions and gaining access to services and organizations and facilitating basic health education groups. The Resource Navigator does not oversee a caseload of patients but acts as a liaison between the patient and case management team and to the program in which they are assigned. On occasion, the Resource Navigator will accompany patients to off-site appointments or may involve driving to patient homes to provide resources for the patient to aid in the navigation of the patient-centered medical home. Resource Navigator does not provide clinical care and does not extend or substitute for the more specialized services of a doctor, nurse, or social worker. Performs other duties assigned.

Qualifications

Education: High School Diploma required. Bachelors Degree in a healthcare or social service related field is preferred.

Work Experience: Experience in a healthcare setting working with patients, healthcare services, medical groups and community resources are required. Experience in direct communication with patients in a high volume fast paced environment preferred.

Cognitive Skills (Language, Math, Reasoning Ability): Excellent verbal and written skills in English and Spanish preferred.

Computer Skills: Microsoft applications required.

Other Skills: Ability to flourish independently and in a team management system. Sensitivity to low income, ethnic minority community. Excellent organizational skills, excellent tracking and documentation required. Excellent attendance required. Excellent listening skills.

Certificates & Licenses: Valid Colorado driver’s license and a dependable and reliable vehicle.

Physical Demands

Frequent standing and walking, occasional long periods of sitting, moderate bending, moderate energy requirements, and frequent fine motor skills. Looking at computer screen continuously for long periods of time, hearing within normal range. Must be able to lift 20 pounds.
