Summary:  
Responsible for assisting patients in achieving maximum wellness through a collaborative, interdisciplinary process involving the patient, family and/or care-givers as well as providers and other clinical support staff. Scope of authority includes providing patient advocacy, resource and referral coordination, follow up, patient education, and self-management education services to patients and to serve as a consultant to medical staff and requesting support from professional staff when required.


This position is for the new Pain and Wellness Clinic located at 340 Printers Pkwy Colorado Springs, CO

Essential Duties and Responsibilities include the following.  
Other duties may be assigned.

  • Assess client psychosocial needs including health literacy status and care deficits (safety concerns and home layout barriers) and makes the appropriate referral for services
  • Assess client's willingness to change
  • Coordinates service planning with client, family, or caregiver to maximize health care responses
  • Facilitates access to quality and cost effective care
  • Coordinate and follow-up on referrals
  • Advocate and build rapport with community agencies
  • Coordinate communication with members of the health care team and educating the client, family or caregiver and care team about treatment options and community resources as well as psychosocial needs and socioeconomic status and deficits
  • Coordinate care with appropriate teams
  • This position may involve driving to client's homes for assessment and follow-up and accompanying patients to off-site appointments as needed
  • Employee demonstrates knowledge of and adherence to the Compliance Plan and conflict of interest requirements. The employee enhances the effectiveness; efficiency and productivity of the department by contributing to, and participating in, departmental performance improve goals.

Suggested Competencies:
Knowledgeable of community resources, sensitivity to low income and diverse groups of people, ability to work with families from a strengths-based model and strong interpersonal skills. Bi-lingual (Spanish) preferred. Self-motivated. Valid driver's license and insured automobile required. Willingness to travel to all PVCHC locations.

Qualifications:

Education/Experience:
Education: Minimum 4 year degree in human service field or related medical field or equivalent experience/licensure required. Prefer Master's degree in human service field or related medical field.

Work Experience:
Three years of experience in case management, crisis intervention or client-centered
counseling and extensive knowledge of community resources required.

Language Ability:
Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents.   Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.

Reasoning Ability:
Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.   Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Computer Skills:
To perform this job successfully, an individual should have knowledge of Word Processing software; Internet software and Database software, Word, Excel, and Electronic Health Record.

Certificates and Licenses:
Current CPR (BCLS) certification preferred
