Description
Peak Vista Community Health Centers is a non-profit healthcare organization whose mission is to provide exceptional healthcare to people facing access barriers.  We provide primary care services (medical, dental and behavioral healthcare) through 26 outpatient health centers in El Paso, Teller, Lincoln, Adams, Elbert, and Kit Carson counties.  We deliver care with our strong “Hospitality” culture.  Our organization has over 850 employees and serves over 10% of the residents in the Pikes Peak region.  Peak Vista is accredited by the Accreditation Association for Ambulatory Health Care, Inc. (AAAHC).

Job Summary (Essential Functions)
  • The Fellowship is a yearlong position which combines mentorship, problem based learning, and supported clinical experience in an outpatient family practice setting
  • Under the mentorship of a preceptor, the Fellows gradually increase the number and complexity of patients seen
  • Specialty, Women’s Health, and Pediatric rotations add to experiential foundations in Family Practice
  • The final six week transition rotation offers multiple pathways for Fellow opportunity beyond the Fellowship experience

Qualifications
  • Active and unrestricted Colorado RN & APRN license (eligible and complete by orientation date)
  • Certification in Primary Care specialty
  • Minimum Master's level nursing education
  • Recently graduated with less than 1800 hours of supervised prescriptive practice, as defined by Colorado DORA
  • Transitioning Nurse Practitioners will be considered on an individual basis
  • Ability to commit to 12-month, full-time program
  • Other, as requested by Peak Vista Community Health Centers
Additional Requirements Documentation
  • Three (3) letters of recommendation including one from your NP Program Director indicating an assessment of your ability to complete your NP program on time and pass FNP certification exam
  • Essay answering the following:
    • What are your professional values as a nurse practitioner?
    • What makes you an ideal candidate for the Fellowship?
    • What are your expectations and goals for the Fellowship?
    • What would your coworkers say are your strongest and weakest personality characteristics?
