Job Summary (Essential Functions)
The Patient Intake Nurse functions as a liaison between new patients and the clinics to ensure all newly enrolled patients establish care. This position will perform routine and emergent nursing functions pertinent to efficient, ambulatory patient care, while following the guidelines set forth by the Colorado Nurse Practice Act.

Duties include: Complete New Patient Screening Summary to include a complete Health History. Assess and triage for the appropriate access to Primary Care based on potential diagnosis and health history. Assess patients readiness for change, physical and psychosocial needs.  Identify and link patients to available resources, social support systems, family support, etc. Facilitate access to quality and cost effective care.  Provide education to patients, families and care-givers. Encourages engagement and self-management of patients with regards to their own health.  Provides outreach to individuals enrolled but never seen in a Primary Care Medical Home. Provides in-reach to patients who are presently inactive in their healthcare. Demonstrates an ability to utilize technology to include, but not be limited to, the Electronic Health Records, state registries, Health Information Exchange.  Maintain documentation of all interactions accurately and completely, implementing appropriate protocols for patients' charts and billing practices.  Maintain healthy working relationship with team members as well as staff from all levels of the organization.  Triage and appropriately delegate follow up needs for patients in regards to labs and any outside testing.  Patient advocate.  Able to provide education as needed to include disease processes and medication regimen.  Work with the care teams to assure schedules reviewed, and patient's chart is prepared for visit.  Demonstrate knowledge of and adherence to the Compliance Plan and conflict of interest requirements.  Enhance the effectiveness, efficiency and productivity of the department by contributing to, and participating in, departmental performance improvement goals. Perform other duties as assigned.

 
Qualifications
Education:
Graduate of an accredited Nursing Program required. Bachelor of Science degree preferred.
 
Work Experience: 
Two years of experience in clinical nursing preferred. 
 
Cognitive Skills (Language, Math, Reasoning Ability):
High language, high reasoning, critical thinking abilities, strong decision making skills; Able to demonstrate organization skills.
 
Computer Skills:
Working knowledge of Microsoft Word, Excel, PowerPoint, and Access software; Ability to perform research on the internet; Ability to use an electronic health record.
 
Other Skills:
Ability to write reports, business correspondence, and procedure manuals.  Ability to effectively present information and respond to questions from groups of managers, clients, customers and the general public. 
 
Certificates and Licenses:
Current Registered Nurse License, current CPR (BCLS) certification.

Physical Demands
