Job Summary (Essential Functions)

Flexible hours is required from 8a-11p to include Saturday and Sunday

Responsible for identifying and responding to the transitional care needs of patients admitted to the emergency department, hospitals and urgent care settings in conjunction with providing exceptional customer service. Provides supervision and support to transitional care  navigators working within community emergency departments or urgent care centers, to include approval of timecards and performance evaluations.    Responsible for resource navigation training specific to transitional care, quality assurance and coordinating/scheduling needed coverage.  Ability to demonstrate a thorough knowledge of all payer sources accepted by PVCHC and external providers. Assist in the development of new and the revision of existing department policies and procedures. Performs other duties as assigned


Qualifications

Education: High School graduate or GED required.

Work Experience: Registered Medical Assistant (RMA) certification or a minimum of 2 years of medical experience and/or community outreach experience required. Previous experience in referral processing desired. 1 to 2 years of supervisory experience preferred.

Cognitive Skills (Language, Math, Reasoning Ability): High language, reasoning and critical thinking abilities. Spanish bilingual preferred.

Computer Skills: Basic computer and keyboarding skills required.

Other Skills: Ability to work with a diverse group of people, handle multiple issues that are time sensitive. Bi-lingual (Spanish/ sign language) preferred.

Certificates & Licenses: Any healthcare certificate or license preferred 


Physical Demands

Frequent standing and walking, occasional long periods of sitting, moderate bending, moderate energy requirements, and frequent fine motor skills. Looking at computer screen continuously for long periods of time, hearing within normal range. Must be able to lift 40 pounds.
