Job Summary (Essential Functions)

Responsible for the management of all functions in the Care Coordination department. This includes all contractual requirements for the productivity of the department(s) and management of High Risk, Preventative, and Transitional Care team.

Essential Duties and Responsibilities include the following:

  • Management or supervisor of employee timecards, PTO requests, evaluations, training and training plans, budgeting and staffing issues.
  • Represents PVCHC as needed in meetings with outside agencies.
  • Collaborates with ACO, RCCO, and numerous payers to ensure compliance with requirements and care coordination activities.
  • Acts as the clinical nursing resource in regards to current and future project management, strategic planning, quality improvement and assurance.
  • Provides leadership and direction for planning, implementation, and evaluation of activities related to the Centralized Care Coordination team.
  • Responsible for maintaining, training and improving interdisciplinary team approach for Care Coordination within the organization.
  • Coordinates interdepartmental processes and workflows in collaboration with other department and leadership teams.
  • Monitors productivity goals and facilitates Process Improvement activities as needed.
  • Responsible for developing, monitoring, and working with teams to report grants and/org contract objectives.
  • Partners with other leadership in the organization to develop and implement protocols to enhance clinical practice and patient services around the Care Coordination workflows
  • Support the Electronic Medical Record systems to assure proper documentation of care coordination activities are reportable.
  • Coordinates with Business Development on community Referral networks.  Reviews agreements and supports relationships.
  • Reviews Quality information and develops and supports activities for improvements.
  • Collaborates with community partners in completing transitions of care for PVCHC patients.
  • Performs other duties as assigned.


Qualifications
 
Education:  Bachelors of Science in Nursing.

Work Experience:  Minimum 3-5 years of nursing experience required with a minimum of 1 year in a nursing leadership preferred; ambulatory care/primary care setting experience preferred; knowledge of process improvement or care coordination or care management preferred.

Language Ability:  Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents.  Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community.  Ability to write speeches and articles for publication that conform to prescribed style and format.

Math Ability:  Ability to work with mathematical concepts such as probability and statistical inference, and fundamentals of plane and solid geometry and trigonometry.  Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.

Reasoning Ability:  Ability to define problems, collect data, establish facts, and draw valid conclusions.  Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.

Computer Skills:  To perform this job successfully, an individual should have knowledge of Word Processing software.

Certificates and Licenses:  Current Colorado RN license.

Physical Demands

The employee must occasionally lift and/or move up to 50 pounds.  Specific vision abilities required by this job includes close vision and ability to adjust focus.  While performing the duties of the job, the employee is frequently required to stand; walk; sit; use hands to finger, handle, or feel and reach with hands and arms.
