Job Summary (Essential Functions)
The Prenatal Coordinator is responsible for engaging at-risk populations of women in El Paso County and educating them on the benefits of prenatal care, providing them with resources and information to access prenatal care and services, and conducting follow-up with them to assess the success of outreach efforts.  Targeted populations include low-income, uninsured, underinsured women, with an emphasis on minority women, women with gestational diabetes and teenagers.  Understanding the eligibility requirements for Medicaid, CHP+ and CICP and is responsible for linking eligible women to enrollment services and programs internally at Peak Vista's Women's Health clinic.  This position is responsible for making best efforts to ensure that the women seen have increased and improved access to prenatal care and assistance with scheduling appointments, decreased difficulty navigating and accessing services within and across systems, increased awareness and education regarding the importance of prenatal care, increased awareness and education regarding the importance of proper nutrition, smoking cessation and dental care during pregnancy and throughout life, appropriate screenings and referrals to mental health services and referrals and linkages to community resources for themselves, their children and their families.  Must exercise discretion and independent judgment to assist clients and families in referral to a health provider specific to the patient's health needs, etc.  Coordinates schedules and teaches a variety of grant sponsored prenatal classes fostering a healthy pregnancy.  Actively recruits, educates and tests Peak Vista established pregnant women for a grant sponsored (Baby Me Tobacco Free) tobacco free pregnancy.  Awards patients with incentive vouchers upon proved "no tobacco" use.  Obtains grant specific data (demographics, birth weights, etc...) and reports them to the appropriate entity and maintains relationships with WIC, PN+, FV and NFP as appropriate.  Performs all other duties as assigned

Qualifications
Education: Professionally trained social worker, nurse, or other human services professional with a Bachelor's degree in his/her field of expertise required.  Registered Nurses who hold a two or three-year degree or diploma may serve as a care coordinator with adequate and relevant experience per the State Prenatal Plus Coordinator.

Work Experience: Minimum of one (1) year of Maternal-Child Nursing, Social Work or Case Management experience preferred.  Knowledge of basic information regarding the normal course of pregnancy, including physical and emotional changes required.  General understanding of basic nutrition, breastfeeding, and the risks associated with the low birth weight/adverse birth outcomes required.  Experience offering prenatal education one on one and in a group setting strongly preferred.

Cognitive Skills (Language, Math, Reasoning Ability): Strong verbal and non verbal communication skills required.  Must have active listening skills, understanding of and commitment to the client-centered approach to working with clients.  High language, high reasoning and critical thinking abilities required.  Bilingual in Spanish strongly preferred.

Computer Skills: Must be computer literate-will need to learn EMP, Web portal, EHR< LabCorp, Physician Link and IRIS systems, keyboarding skills, and Microsoft Office suite.  

Certificates & Licenses: Current license in his/her field of expertise required if applicable. 

Physical Demands
Frequent standing and walking, occasional long periods of sitting, moderate bending, moderate energy requirements, and frequent fine motor skills.  Looking at computer screen continuously for long periods of time, hearing within normal range. Must be able to lift 50 pounds
